Responsible for the promotion, facilitation and support of Transitional Services in the Prison and in the Community.
Identifies new or additional services that are in demand.
Provides the pivotal link of information and advisory services to prison management, staff and offenders, other Government Departments, ‘Not for Profit’ service providers and the community.
Coordinates reintegration supports and facilitates referrals to DOJ Reintegration Contracted Service Providers.

This is a Pool process with permanent and fixed term, full time and part time opportunities.
The Recruitment Pool is open for applications over the next 12 months with suitable applicants offered positions as vacancies arise across locations statewide.

Perform all cleaning duties as required for the designated area following infection control guidelines and relevant policies and procedures.
Transport patients within the hospital in accordance with OH&S guideline and following safe manual handling training.
Assist Nursing staff in patient care (e.g., turns, positioning etc) as required, using the designated equipment, and following the Manufacturer's instructions and Health and Safety procedures.
Maintain confidentiality of all patient and hospital information.
Collect, transport, and store all categories of waste in the correct manner.
Transportation of persons/goods in Q Health vehicle.
